First flighted crew of an Apollo mission went wrong and 3 astronauts died when fire swept into the command center. Communication and oxygen problems were reported before the flight test, they decided to continue. 6:31, oxygen spiked, then one astronaut smelled fire and fire was in the control room. There was risk of the entire spaceship exploding. None of the astronauts made it out.
